  • Demonstration of how the award will support the applicant’s research project and/orprofessionaldevelopment

  • Personal Information Form (see below): Completed and signed by applicant.
  • Statement of Academic and Professional Goals (maximum of 500 words) explaining youracademic and career goals.
  • Description of the research project or professional development activity for which funding isbeingrequested, and how the scholarship will support this work.
  • Resume or CV including academic awards or honors, student organizations, study abroad,internshipsor volunteer work and employment.
  • Academic Reference Form (see below): The form must be completed by university faculty or staffand submitted via e-mail to oge@uic.edu fr om a designateduniversity email address.
